Registration requirements
WHAT ARE THE REGISTRATION REQUIREMENTS?
When registering for school for the first time in Hawaii, a student must present these records:
1. Birth certificate
2. A certificate of release and of proficiency from out-of-state; or any public or private
school in Hawaii
3. Proof of residency in the Nuuanu School district
4. Medical reports
Make certain that your child has the following:
1. A tuberculin (Ma ntoux) test within one year of school entry date. School attendance
is not allowed without proof of freedom from tuberculosis.
When new kindergarten students register in the spring, the health cards must indicate
PPD or Mantoux.
2. A physical examination within one year of school entry date.
3. The complete series of required immunizations.
Increased emphasis is being placed on children receiving all immunizations before
their first day of school.
Immunization required for school attendance shall be those against polio,
diphtheria, pertussis, tetanus, measles (two doses), rubella mumps and hemophilus T
influenza type B disease.
All children entering school should have met the immunizations requirements
before the first day of school. Provisional admittance is allowed only if the parents
shows proof (a doctor's statement or an appointment card for the next vaccination)
that the child is in the process of fulfilling the requirements.
Parents have 7 days to submit this evidence.
